| Short Description of Instrument |
The SF-10 Health Survey for Children is a parent-completed survey that is a shortened adapted version of the Child Health Questionnaire (CHQ). It is made up of ten items regarding the following concepts: Physical functioning, Role/Social Emotional- Behavioral, Role/Social Physical, Bodily Pain, General Behavior, Mental Health and Self Esteem.
The SF-10 has been translated into multiple languages including Spanish, French and Dutch.
